Files shown when no USB inserted

If after unplugging a USB drive from the console you continue to see Phantom Files and the USB appears as if it still mounted:

To fix,

• unplug any USB drives from the console
• go to setup > file manager >USB drive. 
• Use ‘remove’ to delete all files/folders seen.
• Then soft reset the console and the files should be gone  

This occurs when the USB isn't unmounted by the console correctly. In almost all cases removing the files held over and a restart of the console fixes this. We are looking at further ways to ensure this. doesn't happen.
